At Reynolda Smiles we are pleased to offer an in-office dental benefit program for our patients who do not currently have dental coverage. This plan is similar to dental insurance, only smarter! The plan allows patients to receive optimal dental care while maintaining their oral health.
Annual Fees
Individual Plan
$449/per year
Dual Plan (Individual + One Spouse/Child)
$849/per year
Family Plan (Each Additional Household Member)
$849/per year + $389/per added member
What’s Included?
- 2 regular dental cleanings (excluding periodontal procedures)
- 2 dental exams (periodic or comprehensive)
- 1 emergency exam
- Any X-Rays Needed
- Fluoride treatment (One per benefit year)
- 15% discount on general dentistry services (fillings, crowns, bridges, root canals, implants, etc.)
Plan Exclusions & Limitations
This program does not function as a discount plan or dental insurance plan. Instead, it serves as a supplementary option to any existing dental plan. It cannot be used:
- In conjunction with another dental plan.
- For services for injuries covered under workman’s compensation.
- For treatment which, in the sole opinion of the treating dentist or doctor, lies outside the realm of their capability.
- For referrals to specialists.
- For costs of dental care which are covered under automobile medical claims.
This plan is only honored at Reynolda Smiles Family Dentistry and can NOT be used at any other dental office.
Program Guidelines
- You will not receive a membership card – your plan’s effective date will be on file with the receptionist.
- The benefit coverage table is subject to revision annually.
- There will be a $50 reinstatement fee if your plan lapses.
- Cannot be used in conjunction with another dental plan.
- NON-REFUNDABLE. No refunds or premiums will be issued at any time if the participant decides not to utilize the dental plan.
The Dual Plan is for parent/child or husband/wife only. The Dual Plan and Family Plan include any child who is enrolled full-time in college until age 24 or a child who is not enrolled full-time in college until 18.
